What Is the GUIDE Model?

GUIDE stands for Guiding an Improved Dementia Experience.

This innovative Medicare initiative was developed to improve dementia care by providing additional support, education, care coordination, and resources for both patients and caregivers.

The program recognizes an important reality:

Dementia affects the entire family—not just the person receiving the diagnosis.

By providing proactive support and guidance, GUIDE helps families navigate the challenges of dementia with greater confidence and less stress.

Helping People Remain at Home Longer

One of the primary goals of the GUIDE Model is to help individuals living with dementia remain safely in their homes and communities whenever possible.

Through proactive support, care coordination, caregiver education, and access to resources, GUIDE helps reduce crises that can lead to unnecessary hospitalizations or premature nursing home placement.

For many families, that means more time together in familiar surroundings and a better quality of life.

Who Is Eligible?

Eligibility requirements are determined by Medicare and may change over time.

Generally, participants must:

  • Have a dementia diagnosis
  • Meet applicable Medicare requirements
  • Be appropriate for participation in the GUIDE Model
