Hospice care is more than medical care. It is a team of compassionate professionals working together to support patients and families physically, emotionally, spiritually, and practically.
At Hospice Savannah, every patient receives an individualized plan of care built around their unique needs, goals, and wishes. Because no two journeys are alike, no two care teams are exactly the same.
Most importantly, you and your loved ones are at the center of the team. We work alongside patients, families, caregivers, and physicians to ensure every person receives the support they need.
Physicians
Our board-certified Hospice and Palliative Care physicians work closely with your personal physician and healthcare providers to oversee care and ensure comfort.
They provide medical expertise, symptom management guidance, and support for complex healthcare decisions throughout the care journey.
Nurses
Hospice nurses serve as the primary coordinators of care and are often the team members families interact with most frequently.
They assess needs, develop and manage the plan of care, monitor symptoms, provide education, and help ensure patients remain as comfortable as possible. They are also available to answer questions and provide guidance whenever concerns arise.
Social Workers
Serious illness affects more than physical health.
Our social workers help patients and families navigate the emotional, practical, and financial challenges that can accompany serious illness. They provide counseling, connect families with community resources, assist with advance directives, and help coordinate support services.
Chaplains
Our chaplains provide spiritual care and emotional support tailored to each individual’s beliefs, values, and traditions.
They work alongside—not in place of—a family’s own faith community and spiritual leaders, offering comfort, guidance, and a compassionate presence during difficult times.
Nursing Assistants
Certified nursing assistants provide personal care and support that helps patients maintain comfort, dignity, and quality of life.
Services may include assistance with bathing, grooming, dressing, and other daily activities, while also offering companionship and reassurance to patients and families.
Volunteers
Volunteers have been at the heart of Hospice Savannah since our founding.
Specially trained volunteers provide companionship, caregiver respite, emotional support, assistance with errands, meal preparation, transportation, and other practical needs. Their kindness, generosity, and presence often become a source of comfort for patients and families alike.
