CAPABLE®

Helping Older Adults Age Safely and Independently at Home

Most people want to remain in the homes and communities they love as they grow older.

Yet everyday challenges—difficulty bathing, dressing, preparing meals, climbing stairs, or moving safely through the home—can threaten independence and increase the risk of injury, hospitalization, or the need for long-term care.

CAPABLE® (Community Aging in Place—Advancing Better Living for Elders) is an innovative, evidence-based program designed to help older adults maintain their independence, improve daily functioning, and continue living safely at home.

As part of Georgia Care Innovators and the Steward Center for Palliative Care, CAPABLE helps older adults identify goals that matter most to them and provides practical solutions to help achieve them.

CAPABLE is one of several programs developed or adopted by Georgia Care Innovators to help people live well, remain independent, and access support before a healthcare crisis occurs.

A Different Approach to Healthy Aging

Rather than focusing solely on medical conditions, CAPABLE focuses on helping individuals perform everyday activities that support independence and quality of life.

The program is personalized to each participant’s goals.

For one person, that may mean safely getting in and out of the bathtub.

For another, it may mean preparing meals independently, walking safely through the home, or continuing to participate in community activities.

By addressing both personal goals and environmental barriers, CAPABLE helps people remain active, engaged, and independent.

How CAPABLE Works

 

CAPABLE brings together an interdisciplinary team that may include:

  • Occupational therapists
  • Nurses
  • Home modification specialists
  • Community support professionals

Together, the team works with participants to:

  • Identify personal goals
  • Improve mobility and strength
  • Reduce fall risks
  • Increase confidence and independence
  • Address challenges within the home environment
  • Support long-term aging in place

Expanded Support for Greater Needs

For individuals facing more complex health or functional challenges, CAPABLE+ builds upon the success of the CAPABLE model by providing additional support and resources tailored to individual needs.

The program continues to focus on improving function, promoting independence, and helping participants remain safely at home while addressing barriers that may affect overall well-being.

Who May Be Eligible?

Participants generally must:

  • Be age 55 or older
  • Have difficulty performing one or more activities of daily living
  • Be cognitively able to participate in goal-setting
  • Be motivated to work toward personal goals
  • Meet applicable program eligibility requirements
